IMMUNIZATION REQUIREMENTS DUE JANUARY 15TH
The New York State Public Health Law requires certain immunizations for college and other post-secondary students enrolled for at least six (6) semester credit hours or the equivalent per semester. Failure to complete the immunization requirements by January 15th will result in deregistration and removal from housing.

WINTER SESSION SCHEDULE
Health Services is open during Winter session from January 2 to January 17 from 9am
to 4pm for limited medical services. The office will be open from 9am to 5pm for administrative
SPRING STUDENT HEALTH INSURANCE WAIVER DUE 2/15
Students can be excused from the mandatory insurance if they have equal or better health coverage from another US-based company. In order to waive the mandatory insurance for Spring, students must submit an online waiver form at haylor.com/FIT by February 15.
The Health Services is a primary care facility staffed by physicians, nurse practitioners,
and registered nurses. The Health Services Office is funded by the Health Services
fee assessed to students registered for credit-bearing courses who are eligible to
utilize the Health Services Office.
The Health Services provides general medical and gynecological care, and carries a limited supply of medications free of charge to students who paid the Health Services fee. Comprehensive lab tests needed for diagnostic purposes are performed on site by Quest and Labcorp. Any time you are ill, or have questions concerning your health, we are here to help you. We encourage you not only to use the services, but to offer suggestions and comments.
A referral list of consultants is available in cases where a specialist's evaluation and treatment are indicated; however, these and some other services, such as X-rays and lab tests not provided at FIT, are at the student's own expense. Students may consult their health insurance for coverage on these services.
