Non-Credit Instructors

1. Non-Credit Instructor Employment Application 
All employees must complete an FIT employment application. 

2. Background Consent Form
As part of the new hire process, it is the policy of FIT for employees to undergo a background check. In order to initiate this process, you are required to sign this form.

3. Employee Acknowledgment
Complete to acknowledge that you have read and understood the college’s Mission Statement, Strategic Plan, Employee Policy Manual, and Health Insurance Marketplace Coverage Options and Information (.pdf).

4. Employee Confidential Data Form
All new hires are required to complete an Employee Confidential Data Form and Oath of Allegiance. Under New York State law, it mandates employees of public colleges to subscribe and sign the Oath of Allegiance before beginning work. All information obtained from these documents are confidential and will be used internally by the Office of Human Resources.  

5. W-4 Form
Complete this form to ensure that the appropriate amount of federal income taxes are withheld. All employees are required to complete a W-4 form and should consider updating annually or when personal and/or financial matters occur.

6. IT-2104/IT-2104-E
Complete this form to ensure that the appropriate amount of withholdings for New York State (and if applicable, New York City and Yonkers) are withheld. All new hires must complete one of the forms listed below as FIT is a New York State employer. For questions concerning these forms, you may contact the Payroll Office at [email protected].

7. Payroll Allocation
Employees are encouraged to sign up for direct deposit. If you have questions or concerns, please contact the Payroll Office at [email protected].

8. Employment Eligibility Verification Form I-9
Complete Section 1. Employee Information and Attestation  of the I-9 form on or before your first date of hire. This document must be signed at the Office of Human Resources and will be used to verify your identity and authorize employment.
