Workers' Compensation Program
FIT employees are covered under the New York State Workers' Compensation law. This program provides payment for all necessary and reasonable medical care expenses related to your on-the-job injury or occupational disease. This program also provides reimbursement for lost wages at rates set by the New York State Department of Labor.
Immediately report any accident or injury to FIT Security at (212) 2177-7777 and seek
first aid or other necessary medical attention. Security will notify Human Resources
immediately and the employee can expect to hear from a member of the HR Team shortly
thereafter. Please be sure to fill out this form as soon as possible as the HR Team member will require a copy of it.

At the point of injury please call the dedicated Toll Free line: (833) 376-1901. Immediate nurse triage for your injured worker - 24hrs per day/7 days per week/365 days a year. This is a free service for all employees. It is not mandatory but, highly suggested.
