Health Care Plan

Information for: Full-Time Employees

The College offers full-time employees the option to enroll in the Aetna Choice POS II Plan described below, or to waive coverage and receive waiver payments.  New hires have 31 days from their date of hire to enroll in the Aetna Choice POS II Plan or to waive Health Care Plan coverage.  Enrollment in the Plan, if elected, would begin on the 31st day of employment with FIT.  The College pays the full cost of either individual or family coverage*. 

AETNA CHOICE POS II Plan (Open Access)
The Aetna Choice POS II Plan offers you the option of using either In-Network (participating) or Out-of-Network (non-participating) providers each time you access care.  You do not need to elect a Primary Care Physician (PCP) and no referrals are needed for care by Specialists.  When you see an In-Network provider, most eligible services are covered with a copayment of $25 for a PCP and a copayment of $50 for Specialists.  There is no copayment required when you receive services which are preventative.  There is a $25 copayment for laboratory and/or x-ray services.  When you see an Out-of-Network provider, eligible services are paid at 80% of the Reasonable and Customary charge after an annual (calendar year) deductible of $500 per individual (maximum of $1,500 for family) is met.

You may cover a spouse, a same or opposite-gender domestic partner*, and your children under the age of 26.  Along with the health care plan enrollment form, supporting documentation is required when enrolling a dependent such as your marriage certificate, your child's birth certificate, a certificate of registered domestic partnership or alternative documentation if domestic partnership registration is not available where you reside.

Once you are enrolled you may register on Aetna's website to view eligibility, claims history, and discounts available to Aetna members.  Information on downloading Aetna's mobile app is found on the login homepage.  You may contact Aetna's members' services area by calling 1-800-962-6842.

If you are covered by another Health Care Plan, you may elect to waive coverage in FIT's plan and receive a $50.00 per pay period taxable waiver payment from the College (total of $1,200 per year).  To receive waiver payments you must complete a health care plan enrollment form and indicate that you want to waive coverage.

IMPORTANT NOTICE REGARDING ENROLLMENT:  If as a new hire you do not enroll in Health Care Plan coverage within 31 days of your date of hire, you will have an opportunity to enroll in the Plan during the next Annual Benefits Open Enrollment Period which is held in November of each year for coverage which will become effective the following January 1.

If you wish to enroll or add/remove a dependent from coverage outside the Annual Benefits Open Enrollment Period due to a qualified life event as defined by the IRS (e.g. marriage, birth, divorce, loss of other coverage), you must notify a benefits representative in the Office of Human Resources within 31 days of the event.  If notification does not occur within 31 days of the event, you must wait until the next Annual Benefits Open Enrollment Period to make the change which will become effective the following January 1.
