- Faculty Employment Application
All employees are required to complete a FIT employment application. If you have already submitted an application through our applicant tracking system, Interview Exchange, you may disregard this form.
- Background Consent Form
As part of the new hire process, it is the policy of FIT for employees to undergo a background check. In order to initiate this process, you are required to sign this form.
- Employee Acknowledgment
Complete to acknowledge that you have read and understood the college’s Mission Statement, Strategic Plan, Employee Policy Manual, and Health Insurance Marketplace Coverage Information.
- Employee Confidential Data Form
All new hires are required to complete an Employee Confidential Data Form. The data obtained from this document will be kept confidential and used internally by the Office of Human Resources.
Complete this form to ensure that the appropriate amount of federal income taxes are withheld. All employees are required to complete a W-4 form and should consider updating annually or when personal and/or financial matters occur.
Complete this form to ensure that the appropriate amount of withholdings for New York State (and if applicable New York City and Yonkers) are withheld.
All new hires must complete one of the forms listed below as FIT is a New York State employer. For questions concerning these forms, you may contact the Payroll Office at 212.217.3890.
7. Payroll Allocation
To receive payments in lieu of a live check, employees can elect direct deposit. In order to ensure accuracy, it will take up to two pay cycles for direct deposit to go into effect.
All new hires will be required to pick up their first paycheck from the Payroll Office located at 333 7th Avenue, 16th Floor.
8. Employment Eligibility Verification Form I-9
Complete Section 1. Employee Information and Attestation of the I-9 form on or before your first date of hire. This document must be signed at the Office of Human Resources and will be used to verify your identity and authorize employment.
9. Part-Time Faculty Retirement Election Form
All new Part-Time Classroom Faculty and Non-Classroom Faculty must complete this form to elect or decline enrollment in the New York State Teachers’ Retirement System (NYSTRS). If you elect to join the NYSTRS, you must also complete the Membership Application Form and finalize your election by notary prior to submission. If you are unable to notarize the application, you may call the Office of Human Resources ahead of time to schedule an appointment with our in house notary.
