Health Insurance Plan

Information for: Part-Time Employees

FIT offers the Aetna Open Access Elect Choice EPO Plan, an Exclusive Provider Organization (EPO), which provides flexibility in accessing benefits and offers a broad network of participating providers. This plan has In-Network benefits only.  There is no coverage for services provided by a non-participating provider except in the case of emergency care.

Plan features include:

  • Coverage nationally so you can receive care from a participating provider anywhere in the U.S
  • No requirement to designate a Primary Care Physician
  • No referral requirements from a Primary Care Physician to seek care from an In-Network (participating) Specialist
  • No copay for most preventive care services such as routine Adult Physical Exams/Immunizations, Routine Gynecological Exams, and Routine Mammograms

To find an In-Network provider, go to Aetna's DocFind site and enter your search criteria. When prompted to select a plan go to the "Aetna Open Access Plans" section and select "Elect Choice EPO (Open Access)".

For additional information regarding coverage under the Aetna Open Access Elect Choice EPO Plan, please review the Plan Design and Benefits Chart (.pdf), Summary of Benefits and Coverage (.pdf) and the EPO Plan Booklet (.pdf).

Once you are enrolled you may register on Aetna's website to view eligibility, claims history, and discounts available to Aetna members. Information on downloading Aetna's mobile app is found on the login homepage. You may contact Aetna's members' services area by calling 1-800-962-6842.

Please note: Two additional health insurance plan options are available through the UCE of FIT Welfare Trust Fund.  For more detailed information, please view the UCE of FIT Welfare Trust Fund Benefit Booklet (Part-Time Members’ Benefits) (.pdf).  You may also contact Anne Golden at the UCE of FIT Welfare Trust Fund at 212-217-3377.

You may cover a spouse, a same or opposite-gender domestic partner*, and your children under the age of 26. Along with the health care plan enrollment form (.pdf), supporting documentation is required when enrolling a dependent such as your marriage certificate, your child's birth certificate or a certificate of registered domestic partnership or alternative documentation (.pdf) if domestic partnership registration is not available where you reside.

If you do not enroll within 31 days of your date of hire you will have an opportunity to enroll during the next Annual Benefits Open Enrollment period which is held in November of each year for coverage which will become effective the following January 1st.

If you wish to enroll or add or drop a dependent from coverage outside the Annual Benefits Open Enrollment period due to a qualified life event as defined by the IRS (e.g. marriage, birth, divorce, loss of other coverage), you must notify a benefits representative in the Office of Human Resources within 31 days of the event.  If notification does not occur within 31 days of the event you must wait until the next Annual Benefits Open Enrollment period to make the change.
