Student Health Insurance

There is a mandatory hard waiver health insurance requirement. All full-time domestic and international students with 12 credits or more, at least 6 credits of which are on campus, are automatically enrolled and billed for the student health insurance. Graduate full time students (9 credit hours overall) are also automatically enrolled and billed for this health insurance.

Students can be excused from the mandatory insurance if they have equal or better health coverage from another US-based company. In order to waive the mandatory insurance, students must submit an online waiver form at by September 15, 2017.



United Healthcare StudentResources Options PPO, the student health insurance plan, offers:

Insurance Card

The insurance cards are mailed and made available online after the waiver deadline to students who did not waive the school health insurance. If you need the insurance card before the waiver deadline, you need to confirm your enrollment by going to and clicking Full-Time Enroll in Health Insurance. You may download/print your insurance card after your enrollment confirmation has been processed by United Healthcare, around 3-5 business days.





Please submit all claims with patient name and identification number. You may use the Claim Form for reimbursement.

For coverage, present your ID card at the pharmacy when dropping off your prescription. If you have not received your ID card, you may pay for the prescription, then complete the Claim Form for reimbursement.

Medications that are taken for a chronic condition can be filled for up to a 90-day supply through the OptumRx Mail Service Prescription Program. A 90-day supply of a prescription can be filled with a co-payment that is 2 times the co-payment of a 30-day supply.

For students who are traveling abroad and need to fill more than a 6-month supply of a prescription, student must complete the Prescription Override Form.
