Student / Work Study Employees

  1. Employee Acknowledgment
    Complete this document to confirm that you have read and understood the college’s Mission Statement, Strategic Plan, Employee Policy Manual, and Health Insurance Marketplace Coverage Options & Information.
  2. Employee Confidential Data Form
    All employees are required to complete an Employee Confidential Data Form. All information obtained from this document will be kept confidential. 
  3. Federal Withholdings
    Complete this form to ensure that the appropriate amount of federal taxes are withheld from your paycheck. All employees are required to complete a W-4 form and should consider completing a new form each year or when personal and/or financial matters occur.

4. New York State Withholding
All new employees must complete one of the forms listed below as FIT is a New York State employer. For questions concerning these forms, you may contact the Payroll Office at 212.217.3890.

            4a. Employee’s Withholding Allowance Certificate IT-2104

            4b. Certificate of Exemption from Withholding IT-2104-E

5. Payroll Allocation
Complete one of the forms listed below to indicate your preference to receive payment. In order to ensure accuracy, it will take up to two  pay cycles for direct deposit to go into effect. Until then, you should pick up your first paycheck from the Payroll Office located at 333 7th Avenue, 16th Floor.

6. Employment Eligibility Verification Form I-9
Complete and sign Section 1. Employee Information and Attestation of the I-9 form on or before your first date of hire. This document must be signed at the Office of Human Resources and will be used to verify your identity and authorize employment.
