Requirements for New Students

To avoid jeopardizing your enrollment, please complete the following requirements promptly.

 

step 1 Fill out Health Forms online

All students are required to fill out the Health History Form and Notice of Privacy Practices and Consent Form onlineInformation will be kept on file in order to provide health care services to the student during his/her enrollment at FIT. All information is confidential. To fill out the health forms, log in to the FIT health portal with your FIT username and password and click My Forms.

 

step 2 Are you enrolled for at least 6 credits or the equivalent?

Complete the Immunization RequirementsThe New York State Public Health Law requires certain immunizations for college and post-secondary students enrolled for at least six semester credit hours or the equivalent per semester. Students will not be permitted to continue attendance at FIT if these requirements have not been completed. Click here for more details on the immunization requirements.

 

step 3 Are you under 18 years of age?

Your parent or guardian must complete a Parental Medical Consent Form onlineThe Parental Medical Consent Form is required for students under the age of 18. It must be completed in full by student’s parent or guardian. Information will be kept on file in order to provide health care services to the student during his/her enrollment at FIT. To fill out the parental medical consent form, log in to the FIT health portal with your FIT username and password and click My Forms.

 

step 4 Do you have health insurance?

FIT has a mandatory health insurance policy for full-time studentsAll full-time domestic and international students with 12 credits or more are automatically enrolled and billed for the student health insurance. Graduate full time students (9 credit hours overall) are also automatically enrolled and billed for this health insurance. Click here for more details on the student health insurance plan.

Students can be excused from the mandatory insurance if they have equal or better health coverage from another US-based company. In order to waive the mandatory insurance, it is necessary to complete and submit an online waiver form at haylor.com/FIT by September 15, 2018.

FIT takes various measures to protect personal data, including fulfilling its obligations under federal, state, and local law, as well as under the European Union’s General Data Protection Regulation to the extent it is applicable. The GDPR provides protections for various information and records while a person is located in the EU. FIT provides notice to students, employees, and website users of what type of data FIT processes and how and why that data is used and shared through information posted on the GDPR page of FIT’s Privacy website. Please review the applicable notice(s) carefully. The Privacy website also contains useful information about other privacy policies, such as FERPA and Information Technology’s Privacy Policy, and terms and conditions for the use of the FIT website and social media.
