Do not attempt to come to the office in person; applications are accepted by mail only. Without proof your affidavit/application cannot be processed. You must include a self-addressed, stamped legal-size envelope to your current Suffolk address, not the school you are attending.
Suffolk County requires two (2) forms of proof, which must be in student's name. PO Box addresses are not acceptable.
- Proof must be any one of the following showing one (1) year in New York State:
- Selective Service Card
- Voter Registration Card
- High School Transcript or Report Card
- Driver's License or Learner’s Permit
- Auto Registration
- New York State Income Tax Return with W-2 Attached (1040 is Federal tax return; we require NY State – IT-200 or IT-201)
- Plus - one more document dated within the last six (6) months - ie: Car Registration, TAP, Financial Aid, Student Loan, Bank Statement, Credit Card Statement, Phone Bill, or Utility Bill.
If your proof does not fulfill the time frame requirements or if your address does not match current residence, you will be asked to provide additional documents. Please be prepared to furnish additional proof of address. Proof cannot be more than two (2) years old. Any documents issued more than 2 years ago, will not be accepted.
If you are not a US citizen, you must include a copy of the front and back of your green card.
