This plan is a medical and hospital plan which offers you the option of using either in-network or out-of-network providers/hospitals each time you access care. You do not need to elect a primary care physician (PCP) and no referrals are needed for care by specialists. For in-network providers, most eligible services are covered for a co-pay of $25 for a PCP and co-pay of $50 for specialists. When you use out-of-network providers, eligible claims are paid at 80% of reasonable and customary charges after an annual deductible of $500* per individual (maximum of $1,500 for family) is met.

If you are covered by a medical and hospital health insurance plan from another source, you may elect to waive coverage in FIT's plan and receive a $50.00 per pay period waiver payment from the College (total of $1,200 per year). Proof of other health insurance coverage is required.
