Model Permission Forms

Do you need to bring a model on campus who isn't an FIT Student?

You must get permission to bring guests on campus who aren't FIT students.  You need to download a model permission form, fill it out, and email it to the Chair and Assistant Chair at least 24 hours before you plan on bringing your guest on campus. This is the process you need to follow:

Download the form and open it in Preview.

(DO NOT use Adobe Reader! It's the default pdf reader on the lab computers, but it won't let you save the modified pdf)

Fill out all the necessary information. You must include:

Today's date
The shoot date
The shoot beginning and ending time
Your name and the names of your guests

Save the modified pdf your computer.

Email the completed form to both:

Two copies of the signed form will be posted to the door of either D344 or D348. One copy is for the security desk in the C lobby. The second copy is for you to keep with you while your guest is on campus.
